Another nicely designed user interface, this one stark and sophisticated. It offers a number of options and outputs. Easy to use and probably one you would come back to. Also offers a wide variety of sizes for your finished code.

Not as basic as some, but with fewer options of others, the Kaywa QR Code generator does the job simply and effectively.
Sparqcode / Maestro Barcode Generator
Highly developed graphical user interface on this one, although perhaps not as visually impressive as some of the simpler ones. This online code generator will create QR Codes for telephones, SMS, Maps, URL, MeCards as well as raw data.
